List of information needed in order to complete your online application

The following information will be needed when filling out your Scrimmage Fest application.  Please have answers to all the below listed items BEFORE attempting to fill out an application.

General Information Needed

  • Team name, gender, team contact information, age group, and state affiliation
  • The team name should be entered exactly as you would like it to appear in the schedule.

Coach Conflict

  • If your coach has a conflict with another team applying for the Scrimmage Fest, you will need to provide the age group, gender and name of the other team in the comments section of the application.

Competition Level

  • You may select either Premier or Challenge.  Please note that this is just a request.

Contact Information

  • Coach, Team Manager, and Primary Team Contact:  name, address, phone and email for each
  • Email addresses are required, as all correspondence is sent via email.

Team Record (for U13 teams) 

  • Division in which the team played in Spring 2009
  • Team record and final standing (# out of how many teams) for Spring 2009 league play
  • Tournament record and date of tournament for last 2 tournaments, indicating whether your team was the Champion, Finalist, or Semi-Finalist in these events.

Payment

  • Payment for the Scrimmage Fest is completed online at the time of application, using Mastercard or Visa.  Please have your card available before continuing.
