After completing your order form, please fax it to the number indicated on the form. Half of the cost is due at the time of ordering; the other 50% is due upon delivery. If uniforms are ordered by 6/27, you should have them by the end of July. The prices indicated include all embroidery, numbering, and crest.

The numbers will be on back of jerseys, VSA crest will be on right leg of shorts, and "VSA" will be on center front of jerseys. All artwork and requirements are already known by the vendor.
